We currently do not have any vaccine for distribution. We are awaiting notification from the Alabama Department of Public Health as to when we can expect to receive the vaccine.

 

If you have the opportunity to receive the vaccine from another source, we encourage you to do so.

If you are one of our patients, are 65 years or older, or you are one of the frontline workers listed below and would like to be added to our waiting list, please complete the form below. Please do not submit your information multiple times. Please do not call our office to inquire as to if we have received the vaccine. We will update our website, Facebook page, and Instagram page when the vaccine arrives.

You receiving the vaccine is very important to us. We ask that you be patient with us as we follow the guidelines given to us by the Alabama Department of Public Health.
